Opportunity Score: 62/100 · Evidence: Moderate

A drug for a nearby disease might close this gap first This card inherits the parent leftover’s score. It is not a second ranking.

CF-specific anti-inflammatories look empty until you count NCFB DPP1.

Not medical advice, not a recommendation to buy or sell any security, and not a claim that any opportunity should be funded.

HYPOTHESIS

A score or thesis. Always shown with evidence and a way to kill it.

CF-specific industry anti-inflammatories are absent; NCFB DPP1 programs are the adjacent crowding risk.

FACT

A published number with a source and a date. You can check it.

IV-treated pulmonary exacerbations fell to 12.1% (35.8% in 2009) but not to zero. Mean FEV1 is 89.1% among ages >7 and 72.8% among ages 40–49. Sixty-one lung transplants still occurred in 2024 (249 in 2019). Dornase remains 80.6% among ages ≥6.

ANALYSIS

A filter or count this project computed, such as the reviewed CF map.

After excluding NCFB programs, the reviewed CF industry anti-inflammatory map is empty of investigational crowding units. Academic anakinra and losartan are the visible CF-specific bets. N3 whitespace is scored 3, not 5, because late-stage NCFB DPP1 programs (brensocatib-class, including verducatib/BI-1291583 listed not-CF-primary) are adjacent crowding. Ensifentrine and HSK31858 stay off the map.

INTERPRETATION

What we think the fact and analysis mean. Not a score yet.

Residual PEx and impaired FEV1 on HEMT may be inflammation, infection, structural damage, or all three. An empty CF-specific column is not automatically a newco if an NCFB drug can be tried in CF.

Bull case

Why this leftover might be a real product question.

Exacerbations and advanced lung disease did not disappear. If residual disease is still inflammatory, and if NCFB DPP1 does not translate, a CF-specific anti-inflammatory would face an open industry map.

Bear case

Why it might already be solved, or never be a new company.

Effect sizes on an HEMT background will be smaller than pre-modulator inflammation trials. Residual disease may not be treatable inflammation. If brensocatib-class DPP1 works in CF, this thesis is indication expansion, not company formation.

Counterarguments

Reasons a skeptical reader would push back.

  • PEx already fell sharply; the remaining 12.1% may be too mixed to support a new anti-inflammatory.
  • Dornase use shows mucociliary SoC is still used; that is not the same as an anti-inflammatory hole.
  • Anakinra and losartan are investigator-initiated and may never become products.

What would have to be true

Assumptions the bet needs. If these fail, the thesis fails.

Residual PEx and FEV1 impairment on HEMT are inflammatory enough to treat; a CF-specific mechanism or CF trial is required because NCFB drugs do not automatically translate.

Kill the idea if

A pre-committed rule. If this happens, drop the thesis rather than talking yourself into it.

Kill if brensocatib-class DPP1 works in CF or if residual disease is mostly structural damage and infection rather than treatable inflammation.

What to check next

  • Map whether residual PEx on HEMT is inflammatory, infectious, or structural.
  • Watch NCFB DPP1 CF-crossover or CF-specific protocols.
  • Do not score NCFB ensifentrine or HSK31858 as CF crowding; they are already off the map.

Verdict

Watch adjacent. Only pursue a CF-specific inflammation newco if NCFB drugs fail to translate and residual disease is still inflammatory.

This card inherits the parent need score. It is not a second ranking and not a funding recommendation.
