H1 · CF-specific metabolic therapy beyond insulin

Opportunity Score: 77/100 · Evidence: Moderate

Worth a closer look — not a mandate to form a company This card inherits the parent leftover’s score. It is not a second ranking.

Adult CFRD is common, insulin-centric, and empty of CF-specific industry drugs.

Not medical advice, not a recommendation to buy or sell any security, and not a claim that any opportunity should be funded.

HYPOTHESIS

A score or thesis. Always shown with evidence and a way to kill it.

Adult CFRD is common and the reviewed industry drug map is empty.

FACT

A published number with a source and a date. You can check it.

CFRD is 19.2% of the transplant-censored complications population (n≈32,492) and 29.3% of adults in that table. Among people with CFRD, 16.4% had no treatment noted; non-insulin therapies were 8.0%; about 58% of people with diabetes used CGM. Insulin remains guideline standard of care. There is no CF-specific disease-modifying metabolic product on the approved-therapy table. Do not apply 19.2% to the 33,989 demographic total.

ANALYSIS

A filter or count this project computed, such as the reviewed CF map.

The reviewed competitive map has zero industry investigational crowding units on N5. Visible activity is academic: SGLT2, GLP-1, metformin, dorzagliatin, verapamil, plus a bionic-pancreas device program. N5 therefore scores 77/100 on default weights with evidence Moderate, first among the eight needs, because whitespace is empty and adult-era prevalence is high — not because CFRD is a larger causal CFTR gap than N1.

INTERPRETATION

What we think the fact and analysis mean. Not a score yet.

An empty CF-specific drug map can mean a missing product or a problem that generics and insulin already cover. The 16.4% untreated row can be a care-delivery gap rather than a molecular gap. HEMT may still reduce future incidence even if today's adult prevalence is high.

Bull case

Why this leftover might be a real product question.

Adult-majority Registry, countable residual diabetes, and no CF-specific industry drug on the reviewed map. That combination is why this is the default top need. A CF-specific mechanism or formulation would have a measurable glucose endpoint and a possible pulmonary co-benefit.

Bear case

Why it might already be solved, or never be a new company.

Off-the-shelf metabolic agents plus insulin may be enough. Academic empagliflozin, dulaglutide, and metformin studies are the falsifiers. Endpoints that matter in CF (FEV1, weight, exacerbations) are harder than HbA1c. Ranking first is a whitespace result, not proof that a newco should exist.

Counterarguments

Reasons a skeptical reader would push back.

  • Generic incretin and SGLT2 drugs already exist; a CF newco has to beat 'prescribe what endocrinology already uses.'
  • CFF links CFRD to lung-function decline and mortality, but that association does not identify a CF-specific molecular target.
  • The bionic pancreas is a device crowding unit, not a drug, and may absorb insulin-delivery residual without a metabolic newco.

What would have to be true

Assumptions the bet needs. If these fail, the thesis fails.

CFRD remains prevalent on HEMT; generic SGLT2/GLP-1/metformin are insufficient or poorly tolerated in CF; a CF-specific mechanism or formulation has a feasible endpoint (CGM time-in-range or pulmonary co-benefit).

Kill the idea if

A pre-committed rule. If this happens, drop the thesis rather than talking yourself into it.

Kill if academic SGLT2/GLP-1 studies show off-the-shelf drugs close the gap or if HEMT cohorts show CFRD incidence collapsing toward background.

What to check next

  • Read out academic empagliflozin, dulaglutide, and metformin CF trials before assuming a CF-specific mechanism is needed.
  • Check whether HEMT-era pediatric cohorts are still progressing to CFRD at historical rates.
  • Decide whether the product is a CF-specific drug, a CF-labeled metabolic agent, or better insulin/CGM workflow.

Verdict

Pursue diligence on a CF-specific metabolic question. Do not treat the 77 as a mandate to form a company. Kill fast if generics close the gap.
